Lipid Logic: The What, Why & How of Lipid Management

It is axiomatic to target LDL-C levels to lower cardiovascular risk. Over the past four decades, statins have been proven to reduce levels by over 50% and cardiovascular events by up to 30%. However, despite this, there is significant residual cardiovascular risk, LDL-C levels often remain above optimal levels, and there is a significant number of patients who cannot tolerate high-dose statins or any statin. Over the past decade, other lipid-lowering therapies, so-called “non-statins,” have been developed and added to statin or independent of statin (i.e., bempedoic acid), provide additional lipid-lowering and have been shown to reduce the risk of cardiovascular risk even more. We will review the pathophysiologic mechanism for how lipoproteins contribute to atherosclerotic plaque development, briefly discuss how APO B100 and Lp(a) fit in, review serum lipid level goals, and, through cases, become comfortable knowing how to consider and incorporate non-statin therapies into lipid management to lower cardiovascular risk.
